Appointments

Modern General Practice Access

From Monday 8 September we are implementing NHS England’s Modern General Practice Access model. This means that if you request an urgent or routine appointment, we will gather information about your need and a GP will assess how to deal with your request. We refer to this as triage.

Whether you use our online form, telephone us or visit the practice in person, all requests will be dealt with in the same way through our triage system. We recommend you use our online form below to submit your request but if this is not possible you can call or visit reception who will complete the form on your behalf.

When completing the form or speaking with reception, please provide as much detail as possible, including any preferences about how you’d like us to deal with your request. This allows the GP to determine the most appropriate way to deal with your request. The GP may reply with advice, telephone you, signpost you to an alternative provider or invite you for an appointment. Their decision is based on clinical need so we do not guarantee to match your preferences.

Evening and Weekend Appointments

The practice is a member of Gorton and Levenshulme Primary Care Network (PCN). The PCN offers appointments on weekday evenings from 6.30pm to 8.00pm and on Saturday mornings.

The PCN works with Central Manchester Networks to provide these appointments. The appointments are available at 3 practices in the PCN.

  • West Gorton Medical Centre
  • West Point Medical Centre
  • Hawthorn Medical Centre

The appointment may be with a doctor you have not seen before. The doctor will have access to your medical record. We will receive details of your consultation.

Please mention on your online form or to reception staff if one of these appointments would be convenient for you.

Cancelling an appointment

To cancel your appointment:

If you need help when we are closed

If you need medical help now, use NHS 111 online or call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

If you need help with your appointment

Please tell us:

  • if there’s a specific doctor, nurse or other health professional you would prefer to respond
  • if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
  • if you need an interpreter
  • if you have any other access or communication needs

Home visits

If you require a home visit, please notify the surgery before 11:30am, so that the doctors can plan their rounds. Please give the receptionist as much information as possible to enable the doctors to allocate priority to home visits.

Doctors will usually only visit housebound patients or palliative care patients. We will not provide home visits for children or young people.

We can offer you a far better service at the surgery, so please help us to keep home visits to a minimum and attend the surgery.
